Insurance Updates:

Once your healthcare terminates you will be eligible to continue your medical, RX, dental and vision coverage for an addition 18 months at your own expense.

A  COBRA packet  will automatically be mailed out to your home from ADP COBRA Services when your insurance terminates.

If you have exhausted your unemployment benefits, are laid off, available for work and maintained an active registration with MI Works!

you are still entitled to sub. Please continue to turn in your completed sub pay form, (SUB Pay Information Sheet) and documentation to payroll.

Please refer to your layoff letters for termination dates of insurance and brief COBRA information.  COBRA amounts and how to sign up will be coming in the information mailed to your home.

·         Medical/Rx/Dental/Vision Insurance will terminate on «Med_Term_Date». Employees with two to nine years of service will continue to have medical and dental/vision coverage for six months from the end of the month in which you last worked. Once your coverage terminates, you will be eligible to continue your medical and dental/vision coverage for an additional 18 months at your own expense. COBRA forms from ADP COBRA Services will be mailed to your home automatically when your insurance has been terminated.

payment is $365 per week  (plus $10.00 for each dependent up to four). (Per page 4 Sub Benefit Plan Book)

Please make sure you are including your dependents on each of your sub forms.

Some laid off employees have been confusing this number of dependent field with their tax exemption status – it has nothing to do with that.
